Interested contractors are hereby invited to tender for supply of the following categories of Drugs in the Hospital. The Brands and Specifications for these drugs are obtainable on request from the office of the Deputy Director/HOD (Pharmacy).
Group I: ANALGESICS – SYRUP/TABS/CAPS
Group II: ANALGESICS – INJECTABLES/CREAMS
The following conditions must be fulfilled to qualify for consideration
a. The company must be registered to do business with the Hospital’s Drug Revolving Fund for 2013.
b. The company must show evidence of current registration with the Pharmacists Council of Nigeria for the year 2013 ie Registration of premises and superintendent’s annual license to practice.
c. The company must show evidence of tax clearance and pension contribution scheme for 2013.
d. All drugs to be quoted must be NAFDAC Compliant and should have a shelf life of not less than 18 months from the day of supply.
e. The receipt of the tender fee of N2,500.00 ( per group) paid to UBA Drug Revolving Fund account Number 1000546545 must be presented to Pharmacy Department for the collection of tender documents. Any tender document not duly collected from Pharmacy Department renders the bid submitted invalid.
f. A copy of the receipt of the tender fees should be attached to the completed tender documents/quotations etc. They should be properly enveloped, wax sealed and marked at the left corner as “supply of drug group No. etc.
g. The sealed envelope should be addressed to the Chairman Medical Advisory Committee, University of Benin Teaching Hospital, PMB 1111, Benin City and dropped in the appropriate box in the office of the Chairman, Medical Advisory Committee Chairman not later than 12: 05 pm of 19th August, 2013.
h. All completed tender documents must be forwarded to the Chairman, Medical Advisory Committee with a covering letter, written on the company’s letter head.
i. The hospital management is not obliged to accept any lowest tender.
